Louisiana Blue wants to help you on your journey to your best health. Nurses, dietitians, and social health coaches work with you to set and reach personal goals.

Who may be eligible

If you have one or more of the following conditions, you may qualify for this no-cost program:

• Diabetes
• Coronary Artery Disease (CAD)
• Chronic Heart Failure (CHF)
• Asthma
• Chronic Obstructive Pulmonary Disease (COPD)

How it helps

• One-on-one health coaching with Louisiana Blue nurses
• Personalized support and education materials
• Participants are 65% less likely to be admitted to the hospital

Prescription savings

You might save on certain prescriptions. Members may receive select generics at no cost and some name-brand prescriptions for $20 (31-day) / $40 (62-day) / $50 (93-day) supplies.

Eligibility requirements

• Diagnosed with one of the conditions listed
• Enrolled in a Magnolia or Pelican OGB plan from Louisiana Blue
• Not a Medicare-primary retiree
• Agree to connect with a health coach as directed

Louisiana Blue’s Case Management program can help if you have:

• Rare conditions
• High-risk pregnancy
• Organ/tissue transplant
• Serious injuries
• Cancer
• Obesity
• Depression
• Other health conditions
